About the projectThe Connecticut Department of Mental Health and Addiction Services (DMHAS) is expanding and enhancing services for adults with serious mental illness in coordination with new and enhanced supportive housing services in New Haven. DMHAS’s partners include the Connecticut Department of Social Services, the City of New Haven Housing Authority, Advocacy Unlimited, and community-based, supportive housing providers.
Through integrated use of supportive housing, trauma-informed care, primary healthcare, supportive employment, and peer supports, individuals are engaged in comprehensive approaches that are essential for moving toward housing stability, sustained recovery, and self-sufficiency. This initiative increases access to permanent housing for 12 individuals annually and to supportive housing programs for 33 individuals annually in New Haven. |

Continuum of Care, Inc. is a not for profit organization operating licensed group homes, supported living programs, intensive 24–hour supported living programs, community–based crisis and respite services, and residential case management services. Continuum of Care, Inc. is dedicated to providing comprehensive residential case management, crisis services, and housing support services to adults challenged by psychiatric disabilities, developmental disabilities, homelessness, and substance abuse treatment needs while maintaining their rights and dignities as productive members of society.
Through consultation with family members, home healthcare providers, clinical and community providers, roommates and friends, the Continuum of Care staff will develop an Individualized Service Plan that identifies specific personal goals and objectives for the individual's recovery. |
